Patrick Conlon, MSN, CFNP, RN-BC, CDE, CCM, BC-ADM, PNP, FAANP, FNAP

Patrick Conlon - Saint Anthony Hospital Center for DiabetesPatrick is currently the Manager of the Diabetes Education and Management program at Saint Anthony Hospital. He came to us from Mount Sinai Hospital where he was the Director of Diabetes Ambulatory Care, Training & Development.

Patrick has been involved with professional diabetes management and education for over 10 years and belongs to a variety of professional organizations including the American Nurses Association, Sigma Theta Tau, The American Association of Diabetes Educators, and the National Association of Pediatric Nurse Practitioners.

Patrick is also a Fellow of the American Academy of Nurse Practitioners and holds Fellow status with the National Academies of Practice.

He has published in the areas of diabetes management & education and advanced practice nursing. His areas of consultation include: diabetes education & management, as well as exercise and training for chronic disease & weight loss.

Patrick holds a Master of Science Degree in Nursing (MSN) from The University of New Mexico's primary care nurse practitioner program; Bachelor of Science in Nursing Degree (BSN) from Briar Cliff University (Sioux City, Iowa).

He holds board certifications in: Family Practice (as a Family Nurse Practitioner); Pediatrics (as Pediatric Nurse Practitioner); & Advanced Diabetes Management (as a Diabetes Nurse Practitioner).

He is also a Certified Diabetes  Educator (CDE).

Patrick is bilingual (English/Spanish) and enjoys the teaching aspect of his role at Saint Anthony Hospital.

Anna Garcia, RD, CDE

Anna attended the University of Illinois at Urbana-Champaign where she studied Psychology. During her time there, Anna took several nutrition classes and soon became very interested in the connection between food and health. After she graduated with a Bachelors of Science degree in Psychology, she felt that her real interest was in nutrition. Therefore, she decided to go back to school and pursue a career in Dietetics. Anna applied to the University of Illinois at Chicago and was accepted to their competitive Nutrition Coordinated Program. In 2006, she graduated with a Bachelor of Science degree in Dietetics.

Anna’s main interests include diabetes self-management education, weight management, and community nutrition. She currently works at the Center for Diabetes at Saint Anthony Hospital where she teaches patients diabetes self-management education.

Anna is also certified to provide insulin pump training. She has also worked with overweight/obese adults and adolescents on weight loss and/or weight management. Anna has experience in working with people with various education levels, including low literacy.

She speaks fluent Spanish and therefore is able to teach those patients who only speak Spanish.

She is currently a member of the American Dietetic Association and the American Association of Diabetes Educators.
